Individual Access - This Includes only the individual that is eligible for access to the Lemerand Center.
Dual Access - This includes the eligible individual and one additional, dependent family member living at the same residence.
Family Access - This includes the listed individual and two or more (6 maximum) immediate, dependent family members living in the same residence (proof may be required.)
